SYNOPSIS

Failures costs organisation real money and ignoring failures can cost exorbitant amounts of money. Failures is anything that does not met the initial expected or design criteria. In plant equipment, by far the greatest number of failures are item such as bolts and nuts. These items are exceeding cheap to replace compared to do a proper failure investigation. The challenge is to convince the project and users to fund and carry out materials failure investigations.

Fortunately, catastrophic failures are rare in recent years. However, when significant equipment such as pressure vessels, storage tanks, bridges or ships fail, the ramifications can be extensive, in terms of human injury, together with loss of capital revenue. In order to reduce the incidence of costly failures in the future, it is important to learn from past events. These lessons learnt are incorporated into industrial and national codes of practice, and are valuable to practising engineers responsible for the management of critical equipment and plants.

In this webinar, the understand on the vulnerabilities of materials that need to be managed to avoid materials and corrosion failures in project execution and in it’s operate phase will be shared. A few actual past materials/corrosion failures will illustrate the values of performing a proper materials failure investigations. Further, the recently launched IMM’s Materials Failure Investigations Practisioners certification scheme will also be shared.


BIODATA OF SPEAKER

Ir. Ong Hock Guan

Ir. Ong Hock Guan is currently a freelance consultant since March 2024. His last role was the Discipline Head, Materials, Corrosion and Inspection (MCI) Engineering cum Delivery Manager, MCI Engineering/Technical Authority Level 1 since Nov. 2015 based in Menara Shell, KL with a total of 36.5 years working for Shell group.

Ir. Ong has been posted to three (3) overseas assignments in NAM, The Netherlands, The Kashagan Phase 2 project in London under Shell UK and Shell Sakhalin assets in Russia Far East.

He has also been seconded to PETRONAS Carigali Sarawak Operations as Senior Manager for MCI/Production Chemistry/ Asset Integrity. He was also the project manager for Bokor Rejuvention cum the Head, Asset Integrity for the Shell/Petronas JV on the Enhanced Oil Recovery Project (EORC).

Ir. Ong has a Master of Science (Engineering Production & Management) from University of Birmingham, UK. He also has a Bachelor of Engineering (Metallurgy), University of Wales (Swansea), Wales, UK.

Ir. Ong is a Professional Engineer (PE) Malaysia, Chartered Engineer C. ENG. UK, European Engineer EUR. ENG., ASEAN Chartered Professional Engineer (ACPE), Honorary, Fellow Member of Institute of Materials, Malaysia (HFIMM).

Ir. Ong is currently the Institute of Materials, Malaysia (IMM), Honorary Treasurer (2024-2026) and the IMM Corrosion Committee Chairperson (2016-2026).
