SYNOPSIS
In chemical industry, Major accidents have the potential to resultin hundreds of millions of dollars of physical damage, present adanger to employees and the local population, and can lead tosignificant business interruption. However, there are steps thatcan be taken to address Major Accident Hazard (MAH) threatsand minimize the risk of a serious incident as part of acomprehensive Process Safety Management (PSM) program. Afundamental element of PSM, alongside others, such asMechanical Integrity or Management Of Change (MOC), isProcess Hazard Analysis (PHA), a key tool for understandingMAHs. This technical talk on PHA Revalidation will provide guidance tovalidate the control barrier and evaluate the control and recoverybarrier validity and its effectiveness, and eventually determine theadequacy in achieving ALARP. This will assist participants betterunderstand of ways to facilitate the HAZOP and make morefirmed decision on the adequacy of safeguards.

Ir. Mohamad Rosli is thePSM Head for DownstreamProject, PD&T in PETRONAS.He has more than 15 years ofProcess Safety experience inOil & Gas industry. His rolealso includes ProjectTechnical Authority (PTA) forthe project supervised byhim. Registered with BEM as Professional Engineer withPractising Certificate (PEPC). IrMohamad Rosli hold abachelor degree of ChemicalEngineering from LouisianaTechnology University, USA.
Mr. Amirzudin Mizuar
is the PSMHead for Asset, PD&T inPETRONAS. He has more than 20years of Process Safety experiencein Oil & Gas industry. Involved inthe establishment of PETRONAS'Process Safety Managementsystem, as well as numeroussafety studies for PETRONAS andexternal parties' operating facilitiesand projects. Amirzudin Mizuarholds a Bachelor Engineeringdegree in Chemical Engineering,from University Science Malaysiaand Master Engineering inEnvironment Engineering fromUniversity Kebangsaan Malaysia.
